Myocardial Iron overload quantification in a developing country: Tunisian first experience with financial challenges

چکیده
Background Iron chelation treatment has increased life expectancy of patients with beta thalassemia major. Accurate assessement of heart iron overload is essential for managing chelation therapy. Nowadays, MRI-based iron overload quantification became the gold standard technique. However it remains too expensive in countries that cannot afford such technology, which ironically are endemic countries of thalassemia. The aim of our study is to evaluate the impact of measuring heart iron concentration by the means of MRI using a “low cost” technique in a developing country like ours with few financial resourses.

Assessment of Pancreatic Iron Overload in Transfusion Dependent Thalassemic Patients
Advances in the management of transfusion dependent thalassemic patients have improved the survival of these patients. The most important consequence of repeated and frequent transfusions is iron accumulation in vital organs. The magnetic resonance imaging (MRI) is a non-invasive and valid technique for the estimation of iron stores. Despite multiple studies about cardiac and liver MRI T2*, the...
متن کاملComparison of iron chelation effects of deferoxamine, deferasirox, and combination of deferoxamine and deferiprone on liver and cardiac T2* MRI in thalassemia maior
Background: Cardiac complications due to iron overload are the most common cause of death in patients with thalassemia major. The aim of this study was to compare iron chelation effects of deferoxamine, deferasirox, and combination of deferoxamine and deferiprone on cardiac and liver iron load measured by T2* MRI. Methods: In this study, 108 patients with thalassemia major aged over 10 years...
متن کاملFat Content Effect in the Measurement of T2* for Iron Quantification in the Liver in Patients With Suspected Myocardial Iron Overload
Background Iron quantification using T2* has become a routine noninvasive procedure for assessment of myocardium and liver in patients with regular transfusions. Cardiologists and cardiovascular radiologists are progressively measuring iron overload in the liver associated with myocardial overload. It is currently unknown if potential interference of high fat content in the liver may result in ...
متن کاملMyocardial iron quantification using modified Look-Locker inversion recovery (MOLLI) T1 mapping at 3 Tesla
Background Quantification of myocardial iron overload is critical for the management of patients with hemochromatosis. The effects of excess iron on T1 and T2* relaxation times correlate directly with tissue iron concentration. T2* became the clinical standard at 1.5T as it can be easily obtained in a fast one breath-hold ECG gated multi-echo GRE sequence. At 3T, however, T2* quantification can...
متن کاملEvaluation of the relationship between hepatic and cardiac iron overload with MRI T2* and carotid intima media thickness with Doppler ultrasound in beta thalassemia major patients
Background: Iron overload is caused early progression of atherosclerosis in beta thalassemia patients due to regular repeated blood transfusion. MRI T2* is a gold standard non-invasive method for detecting hepatic and cardiac iron overload. The aim of this study was the comparison of carotid intima media thickness (CIMT) in the patients and healthy control groups with Doppler ultrasound for ear...
